Please, follow the steps outlined below to format a Polycom phone to factory defaults:

Press the Home or Menu button
Select Setting > Advanced
The password is going to be either 2580 or 456
Select Administrative Settings
Select Reset to Defaults
Select Reset to Factory
Then Yes to confirm
The phone will reboot a few times and should re-register on our network. It will take approximately 10 min.
